POSITION SUMMARY: Responsible for the completion of functional assessments, coordinating appropriate community-based services for functionally impaired older persons, and assisting in the ongoing evaluation, development, and sustainability of quality improvement processes to ensure the highest of quality standards.

 

Wage: $39,900.00/yr.

ESSENTIAL JOB FUNCTIONS:

  • Responsible for knowledge of all formal and informal community resources.

  • Utilizes client centered principles to facilitate coordination of services which supports client self-sufficiency and focuses on continuum of care.

  • Conducts telephone calls/client home visits as required to assess client safety/needs and to ensure services are adequately being provided.

  • Ensure all assessments, care plans, narratives, and referrals are completed timely and meet required quality standards.

  • Ensure client case records are continually updated and meet quality standards.

  • Complete ongoing training requirements as directed.

  • Work with clients to develop an informal support network (relatives, volunteers, friends, etc.) as needed.

  • Utilizes data and other sources of information to make recommendations for process improvements.

  • Performs regular observations and monitoring functions as directed to ensure overall programmatic compliance.

  • Assist in the development of an annual training plan and provides ongoing in-service training to staff as needed.

  • Assists in the development, implementation, and sustainability of Best Practice standards.

  • Serves as a support to Director/Service Area Managers on all matters related to training and quality assurance/improvement to include but not limited to CIRTS cleanup and outcome measures reports.

  • Attends and actively participates in training programs, staff meetings, and other meeting/trainings as directed.

  • Maintains confidentiality in all aspects of client, staff and agency information.

  • Maintains professionalism, a positive image, and effective working interactions with co-workers, clients, and all outside contacts within the community.

  • Uses and follows the policies/procedures of Mid Florida Community Services, Inc., including but not limited to Personnel Policies, Occupational Health and Safety policies, payroll policies/practices, etc.

Performs any additional duties as directed or assigned by supervisor, management staff, program director or Mid Florida Community Services, Inc. management staff. Requirements

JOB STANDARDS:

Education/Experience: Bachelors Degree in Human Services or related field and four years Senior Services Case Management experience. An equivalent combination of education/experience may be considered.

Licenses & Certifications: Valid Florida drivers license and be insurable by Companys current carrier. Subject to Federal, State and Local legal requirements/background checks/clearance for working with a vulnerable population.

 

CRITICAL SKILLS, ABILITIES, & EXPERTISE:

Physical Requirements: These physical demands are representative of the physical requirements necessary for an employee to successfully perform the essential functions of the position. Sitting, standing for long periods of time, use of fingers, arms, hands and legs and voice/talking are constant. Good eyesight (correctable) and hearing (correctable) are essential. Squatting, walking, handling, grasping, stretching/reaching, bending at the waist, driving and light lifting and/or carrying (up to 15 lbs.) are frequent. Pushing, pulling, kneeling, balancing, turning, feeling, medium lifting and/or carrying (up to 50 lbs.) are occasional.

Equipment: Computer, facsimile, copy machine, calculator, multi-line phone and other small office and instructional equipment and vehicle.

Skills & Expertise: Ability to work with limited direction. Knowledge of organizational methods. Skills in completing work with a high degree of accuracy. Ability to effectively communicate orally and in writing. Ability to establish effective working relationships with people, including supervisors, peers, vendors, and clients. Ability to compile data from various sources, departments, etc. Expertise in the operations of Word processing, and Window based applications. Ability to use and operate personal computer and general office equipment. Skills in customer service and dealing with difficult individuals. Ability to work in a multi-task environment. Ability to maintain client and staff confidentiality.

 

ENVIRONMENTAL JOB FACTORS:

Job Location: Various locations within Sumter, Lake and Hernando Counties. Position can be relocated at any given time for any reason with sufficient notice provided. Occasional travel to other counties may be required.

Work Environment: While performing the responsibilities of the position, these work environment characteristics are representative of the environment the employee will encounter. Office environment and working with others frequently. Working with office equipment is frequent. Occasionally outdoors and operates a vehicle.
